07-08 Data Surveys

Parish Catechetical Data Survey 2007-08 was mailed the third week of April and is due completed by May 23, 2008.

Please fill it out and return it by email, mail or fax as soon as you have all of the needed data gathered. Deadline: May 23.

You can download the survey with form fields in Microsoft Word format. You can fill out this version by tabbing from field to field right on your computer.

DO NOT JUST OPEN THE DOCUMENT IN YOUR BROWSER TO FILL OUT AND SEND WITHOUT SAVING IT FIRST. THAT WOULD SEND A BLANK DOCUMENT TO US AND YOU WOULD LOSE ALL OF YOUR DATA ENTRY!

Double-Click on the icon for the downloaded data survey to open in in Microsoft Word.

Your cursor will automatically be set at the first "blank" to be filled in. Enter will move your cursor from one blank to the next. You also can use your mouse to set the cursor in any field you choose.

Fill in all of the necessary data. If you need to fill it out partially and continue later, save what you have finished before closing the document. Please consult others for data you do not have at hand.

When the form is complete, save it to your hard drive for your own record. Print it out for your own hardcopy.

To deliver the survey to the OFC, you can:

	send it as an email attachment to OFC@archchicago.org OR
	Print it and mail
